You must be at least 18 years of age to book an appointment or receive services at Everis Aesthetic Medicine. If you are under 18, a parent or legal guardian must provide written consent and be present during the consultation and treatment.

Medical Disclaimer

Everis Aesthetic Medicine provides aesthetic medical treatments including, but not limited to, Botox, dermal fillers, laser hair removal, chemical peels, and skin rejuvenation procedures. All information on this website is provided for general informational purposes and does not constitute medical advice.

Consultation and Informed Consent

All treatments at Everis Aesthetic Medicine require a prior consultation with one of our physicians. During this consultation, your physician will evaluate your medical history, discuss treatment options, and explain potential risks and benefits.

Appointment Policy

Scheduling

Appointments may be scheduled by phone at 408-899-8771 or through our online booking system.

Cancellation and Rescheduling

We ask that you provide at least 24 hours notice if you need to cancel or reschedule an appointment. Late cancellations or missed appointments may be subject to a cancellation fee. Repeated no-shows may result in a requirement to prepay for future appointments.

Late Arrivals

If you arrive more than 15 minutes past your scheduled appointment time, we may need to reschedule your appointment to ensure adequate treatment time and avoid delays for other patients.

Payment Terms

Payment Methods

Everis Aesthetic Medicine accepts major credit cards, debit cards, and cash. Payment is due at the time of service unless an alternative payment arrangement has been agreed upon in advance.

Insurance

Aesthetic medicine treatments are generally considered elective and are not covered by health insurance. Everis Aesthetic Medicine does not file insurance claims for cosmetic procedures. Patients are responsible for the full cost of treatment.

Refund Policy

Due to the nature of aesthetic medical services, refunds are not available once a treatment has been performed.
